AbstractDuring a 4-year operating interval with at least a one-year follow-up period, 28 deep infections occurred out of 946 reamed femoral intramedullary nailing cases (3.0%). 25 deep infections were treated with debridement, local drainage, and retention of intramedullarynails. There were 2 infections extending to hip pyarthritis (8.0%) during the follow-up period. Both cases had a follow-up period of 4 years. Both fractures healed uneventfully but the hip joints deteriorated insidiously and degeneration occurred. The joint infection proceeded so insidiously that detection and management were easily disregarded. Routinely aspirating the hip joint while debridement of the femoral shaft was suggested. To avoid joint degeneration, an immediate aggressive joint debridement might have to be done once the hip joint was involved.
