| Abstract | The assessment phase of the individualized living planning process helps team to learn about the individual and to obtain the information needed to develop an effective and comprehensive individualized living plan. Through functional and individualized assessment, the team determines the individual's capabilities and needs, as well as services which will support the individual. Assessment information also provides the data for the development of the individualized living plan. This article describes the ways of assessing the plan. |
