AbstractDiagnosis of malaria often requires high index of suspicion. Patients may have uncommon presentation and mode of transmission. This article reports a patient in Hong Kong diagnosed Plasmodium falciparum infection three weeks after receiving platelet transfusion and prednisolone for idiopathic thrombocytopenic purpura. She had a history of travel to Thailand two years before 1996. On the other hand, the platelet donor had a raised antibody titer by the falciparum-spot IF test which could signify both an acute and a past infection. People could not conclude whether this patient had an unusally long incubation period of P. falciparum infection unmasked by immunosuppressive therapy or malaria from platelet transfusion. Evidence available for both situations were discussed.
