AbstractA high interrater diagnostic reliability and objective measurements of therapeutic response are essential in drug trials of Alzheimer's disease (AD). Most therapeutic trials on AD are based on the cholinergic hypothesis. Although only modest clinical improvements are observed, future developments in cholinergic therapy have a great potential. Some of the other therapeutic strategies such as the use of metabolically active compounds also produce small but consistent response. Investigations on these drugs might provide other hints for the neuroendocrinological aspects of the aetiology of AD. Psychosocial aspects of management are also important and much of the load will rest on the primary health carers.
