AbstractLeft ventricular hypertrophy (LVH) is a commonly encountered complication of hypertension which has important prognostic implications. The histopathological changes in this disease are not only characterised by the presence of myocytes hypertrophy, but also importantly by the presence of myocardial interstitial fibrosis. The pathogenesis of myocardial interstitial fibrosis is complex and is not fully understood. It is postulated that the presence of hypertension will trigger the onset of a cascade of fibrogenesis, which will involve mechanisms such as mechanical stretching, activation of local renin-angiotensin system and increased cytokine gene expression such as transforming growth factor-[B] (TGF-[B]). Understanding the pathogenesis of myocardial fibrosis may have important clincial implications as future treatment modalities should aim at prevention or regression of left ventricular hypertrophy and fibrosis.
